What is CHIP

Using what we learned from the Community Health Needs Assessment (CHNA), our stakeholders and partners work together to develop strategies to address issues facing the community. The strategies are considered using the following criteria:

  • Addresses inequities among specific groups and communities
  • Data and trends can be monitored over time using a shared approach 
  • Requires collaboration and includes multiple sectors 
  • Involves program and system changes 
  • Demonstrates alignment with Healthy Ashtabula County’s mission and vision

From this process, the Community Health Improvement Plan (CHIP) was developed. The CHIP provides the goals, objectives, and strategies to improve health and well-being in the county.
